	I/my company agree/agrees to host a party before October 1, 2009 for the purpose of supporting the Pink Bedroom Party 2009. I/my company will collect $30 - $50 from each of my guests (friends, families, colleagues). 50% of the proceeds contributed to me/us or venue of my/our choice, 50% donation will be payable to Jeanne Lottie one week after the party. All the net proceeds of the party will be donated to The Campbell Family Cancer Research Institute at The Princess Margaret Hosiptal, SickKids Hospital, or my/our designated charity. Please enter the charity name:

	I/my company agree/agrees to have a party minimum of 20 people or minimum donation of $300.

	I/my company agree/agrees to use the Pre Pink Bedroom Party template to invite my/our friends. The invitation has to be approved by the organizer and the charity.

	I/my company would like to get a free ticket to the main event on Oct. 1, 2009, value at $50. Please send the ticket to:

	I/my company would agree to waive any organizer’s liabilities caused by my/our party for irresponsible drinking and other mishaps. (Please be a responsible host.)

	I/my company would not use the trademark Pink Bedroom Party without the consent from the organizer, each party has to be registered with acknowledgement from the organizer.
